What is the Medicare Prescription Payment Plan?

In 2026, the Medicare Prescription Payment Plan helps beneficiaries manage high prescription costs by spreading out-of-pocket (OOP) expenses into capped monthly payments.
Key Changes for 2026

  • Increased OOP Cap:​ The annual maximum you will pay for covered Part D drugs is $2,100 in 2026.
  • Higher Deductible: The maximum annual deductible for Part D plans is $615 in 2026
  • Automatic Renewal: If you were enrolled in the payment plan in 2025 and stay with the same insurance plan for 2026, you will be automatically re-enrolled.
  • Negotiated Prices: For the first time, Medicare-negotiated lower prices for 10 specific high-cost drugs takes effect, which may further reduce your total costs.

How the Payment Plan Works
  • $0 at the Pharmacy: Once enrolled, you pay nothing at the pharmacy counter when picking up covered medications.
  • Monthly Billing: Your Part D or Medicare Advantage plan pays the pharmacy and then bills you monthly.
  • The Calculation: Your monthly bill is calculated by taking your total drug costs (plus any remaining balance) and dividing it by the number of months left in the calendar year.
  • Flexible Monthly Amounts: Because you have fewer months to "spread" new costs as the year progresses, your monthly bill may fluctuate.

Is It Right for You?

This plan does not lower your total costs; it only changes when you pay them. It is best for people who hit the $2,100 cap early in the year (before September) or who take high cost medications.
It is not recommended for those with low or steady monthly drug costs, or those who qualify for Extra Help (Low-Income Subsidy), as they already have low copays.

The Payment Plan is Voluntary

To opt in to the payment plan, you will need to go to your plan's website, or call them. Since the payment plan is voluntary, you can enroll and disenroll at any time. If you disenroll partway through the year and still owe a balance, it will have to be paid in full or you will continue making a monthly payment. You will then pay for your medications when you pick them up at the pharmacy. If you change plans and switch to a Medicare Advantage plan during Open Enrollment, you must opt-in to the payment plan again with your new provider.

Other Considerations

Your monthly bill is not a fixed amount. If you add new prescriptions later in the year, your monthly payment will likely increase because there are fewer months left to spread out the new costs.
Additionally, the plan only applies to Part D covered drugs. Any costs for Part B covered drugs (like some injections) or drugs not covered by your plan must still be paid at the pharmacy.
