Some individuals qualify for both Medicare and Medicaid at the same time, offering robust healthcare support. These individuals are often referred to as "dual eligibles." Let's break down exactly who is eligible for both Medicare and Medicaid.
Understanding Medicare and Medicaid

Before diving into dual eligibility, it's important to understand these programs. Medicare is a federal health insurance program primarily for people aged 65 or older, individuals with certain disabilities, and those with End-Stage Renal Disease (ESD). Medicaid is a joint federal and state program that provides health coverage for individuals with low income and limited resources, including some older adults, children, pregnant women, and people with disabilities.

Dual Eligibility Criteria

To qualify for both Medicare and Medicaid, individuals must meet the eligibility requirements for each program.

Medicare Eligibility: 
  • Age 65 or Older: Most people qualify for Medicare when they turn 65 if they've worked and paid Medicare taxes for at least 10 years.
  • Younger Than 65 With Disabilities: Individuals who have been receiving Social Security Disability Insurance (SSDI) for 24 months are eligible.
  • End-Stage Renal Disease (ESRD) or ALS: Individuals with ESRD or Amyotrophic Lateral Sclerosis (ALS) automatically qualify.
Medicaid Eligibility:
  • Income and Asset Limits: Medicaid eligibility is determined based on income and, in some states, resource limits. These limits vary by state and by the specific Medicaid program.
  • Categorical Requirements: Medicaid also has categorical requirements, meaning applicants must fit into certain groups, such as being aged, blind, disabled, or a child under a certain age.

Types of Dual Eligibles

Dual eligibles can fall into two different categories based on their level of Medicaid support. The first category is Full-Benefit Dual Eligibles. Full-Benefit Dual Eligibles (FBDE) receive full Medicaid benefits, including long-term care, dental, vision and transportation services. Medicaid pays for Medicare premiums, deductibles, and copayments. The second category is Partial-Benefit Dual Eligibles. Partial-Benefit Dual Eligibles receive assistance with Medicare premiums and cost-sharing through Medicaid programs such as the Qualified Medicare Beneficiary (QMB) or Specified Low-Income Medicare Beneficiary (SLMB) programs.

Benefits of Dual Eligibility

Dual eligibility provides significant financial and healthcare advantages such as:
  • Comprehensive Coverage - Access to both Medicare and Medicaid services
  • Reduced Out-of-Pocket Costs - Medicaid may cover Medicare premiums, deductibles, and copayments.
  • Long-Term Care Support - Medicaid covers services like nursing home care and home and community based services that Medicare doesn't.
  • Prescription Drug Coverage - Dual eligibles automatically qualify for Medicare Part D's Extra Help program, significantly lowering prescription drug costs.

How to Apply for Dual Eligibility

You can apply for Medicare through the Social Security Administration (SSA) online, by phone, or at a local office. Medicaid can by applied for through your state's Medicaid office. Each state has its own application process and eligibility criteria. If you are eligible for both programs (Medicare and Medicaid), you can apply for the Extra Help program for prescription drug costs through the SSA.

Dual eligibility for Medicare and Medicaid ensures that some of the most vulnerable populations receive the comprehensive healthcare they need. Understanding the qualifications and application processes for each program is the first step to unlocking these benefits.
